As the county-owned skilled nursing facility, we have been a fixture in our community for more than 50 years, delivering exceptional care to the residents of Fond du Lac County and the surrounding areas. Harbor Haven Health & Rehabilitation has become the area’s leader in providing short-term rehabilitative services to those in need of post-acute rehab care.

Located just behind the city’s largest in-patient hospital and surrounded by ample green space with beautiful gardens, our facility features five state-of-the-art households. Our two short-term rehab and two long-term care households, along with our dementia care household, deliver specialized care and services. Our dedicated and compassionate team collaborates closely with our residents and their families, guiding and supporting them every step of the way.

Harbor Haven is a member of LeadingAge Wisconsin, a network of not-for-profit long-term care providers throughout the state of Wisconsin. LeadingAge Wisconsin supports the advancement of its long-term care provider members in an effort to improve the lives of those individuals who are served and cared for in these long-term care environments.

Dementia Specific Facility

LeadingAge Wisconsin believes that facilities should be recognized for providing the utmost quality of care to individuals living with dementia. That’s why they’ve given Harbor Haven a Dementia Specific designation. This means our care community is dedicated to a holistic philosophy that embraces and fosters individualized dementia-centered care and support.

Eligibility

Licensed as a skilled nursing facility, Harbor Haven is Medicare and Medicaid certified. We also accept private payment and commercial insurance. Eligibility is not limited to residents of Fond du Lac County.

I wanted to take a moment to thank you and your staff for the wonderful family picnic that Harbor Haven hosted for your residents and families. Everything was just perfect! The food was great. The theme and decorations (even in the elevator) really made the event fun. The centerpiece flowers were adorable. The cute ribbon on the flowers really tied the whole theme together. The music outside was so good. They could really sing! Your staff was so friendly and accommodating. I saw them interacting and dancing with the residents. Your staff has a hard job. They need to know they do make a difference for many families. I know we are forever thankful to all of you.
