Palliative and Hospice Care

Palliative and Hospice care are provided at Harbor Haven Health & Rehabilitation by an interdisciplinary team of physicians, nurses, nursing assistants, social workers, and behavioral health professionals. Harbor Haven does contract with some local Hospice providers. The goal of hospice or palliative care is to provide support to residents and their loved ones that face end-of-life, chronic, or life-threatening health conditions, and to offer the highest quality of life that is attainable. Palliative nursing home care addresses not only the resident's medical needs, it carefully considers the psychological, emotional, and spiritual needs of these residents as well. Our staff is trained in and has extensive experience in addressing the special needs of individuals at this stage of life, providing support to residents in our care as well as family and loved ones.
